One question that I think personally needs to be looked at was the lifting loads at height he was stood half way up the step ladder with a load but he had no safe route down off the ladder as he had his hands full and would not be able to hold onto the ladder to get down and he is also unable to twist to pass the load as that’s a danger of hurting your back
